From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

(-23) While on approach to Toledo Express Airport "tol" airman encountered gusty conditions (200 12G18) on final to runway 25. As the aircraft was in the flair, with 20 degrees of flaps and a speed of approximately 80 KTS, airman stated the wind or gust had died off and the airplane just fell out of the sky and struck the runway. The airplane bounced multiple times and could have possibly struck the nose gear and propeller, but wasn't sure. Airman could not control the aircrafts directional control when she slowed the aircraft down to taxi off the runway. The aircraft made a left uncommanded turn bringing the aircraft's longitudinal axis approximately 30 to 40 degrees left of runway centerline. ATC advised airman she had a flat tire. Aircraft damage was minor with propeller damaged due to propeller striking the runway and nose wheel tire assembly blown with rim broken apart.
