From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

N921AL departed 2LS0 at 0602 in route to WR206 to exchange passengers. After landing a passenger unload and load was conducted without shutdown. The flight was uneventful for the return to 2LS0 with a safe landing at 0850. A post flight inspection revealed that an APU access door/panel located horizontally on the top side of the aircraft had departed during the flight. It was also determined that the door/panel had made contact with a main rotor blade, the tail boom, and a tail rotor blade.
