From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. When the aircraft was at an approximate altitude of 150-200 feet, it turned to the right, pitched down, and descended vertically to the ground. The aircraft was destroyed and the pilot and three passengers were fatally injured. The planned destination for the flight was unclear during the investigation due to conflicting information. It may have been provo, UT (pvu) or Wendover, Ut (env). The pilot requested assistance from air traffic control after becoming disoriented and was located 25 miles south of Wendover, Ut in a restricted area so was directed to Wendover, Ut.
