From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

On June 26, 2012, UH1F helicopter, N4582D, was involved in an accident injuring a ground worker while conducting external load operations in close proximity to high voltage transmission lines. N4582D was positioning a Concrete bucket over a high tension power line tower foot pad while Concrete was being poured. During the pour, an electrical charge from the adjacent transmission line arced to the synthethic long line and traveled down the remote hook electrical lead releasing the load and energizing the bucket. A ground workman pouring Concrete completed the circuit and suffered severe electrical burns requiring hospitalization. There was no injury to the pilot or damage to the helicopter.

As the FAA filed it

A GROUND WORKMAN POURING CONCRETE COMPLETED THE CIRCUIT AND SUFFERED SEVERE ELECTRICAL BURNS REQUIRING HOSPITALIZATION.THERE WAS NO INJURY TO THE PILOT OR DAMAGE TO THE HELICOPTER.
