From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

privacy data omitted Private pilot 3452258, was the pilot in command of N2895L when it departed the taxiway and impacted a taxi sign at the intersection of n and N3 taxiways. privacy data omitted Was out on a 0.6 hrs instructional flight in the pattern at atw airport with privacy data omitted(flight instructor) prior to the incident. She dropped him off on the ga ramp, and was headed out to do some solo pattern work. She requested taxi to rwy 12, her instructions were to taxi to rwy 12 via November "n" and bravo "b" and instructed to hold short of rwy 12. According to privacy data omitted she negotiated the left turn but could not turn right, which resulted in her departing the taxiway and impacting the taxi sing. privacy data omitted Stated the right barke pedal would not move any further and appear to be ineffective. Inspector daniel r. oskar checked the brakes in both pilot position, and they were functional. The nose wheel steering also appeared to operate correctly. Inspector oskar questioned privacy data omittedand her instructor to see if either one of them was aware of the horizontal bar above the rudder pedals in the PA-28R-201 aircraft and they were not. It is not uncommon for pilots to get their toes on this bar when trying to apply the brakes, rendering the pedal(s) inefective. It appears that this was the cause of the excursion off the taxiway.

As the FAA filed it

^PRIVACY DATA OMITTED^ PRIVATE PILOT 3452258, WAS THE PILOT IN COMMAND OF N2895L WHEN IT DEPARTED THE TAXIWAY AND IMPACTED A TAXI SIGN AT THE INTERSECTION OF N AND N3 TAXIWAYS. ^PRIVACY DATA OMITTED^ WAS OUT ON A 0.6 HRS INSTRUCTIONAL FLIGHT IN THE PATTERN AT ATW AIRPORT WITH ^PRIVACY DATA OMITTED^(FLIGHT INSTRUCTOR) PRIOR TO THE INCIDENT. SHE DROPPED HIM OFF ON THE GA RAMP,AND WAS HEADED OUT TO DO SOME SOLO PATTERN WORK. SHE REQUESTED TAXI TO RWY 12, HER INSTRUCTIONS WERE TO TAXI TO RWY 12 VIA NOVEMBER "N" AND BRAVO "B" AND INSTRUCTED TO HOLD SHORT OF RWY 12. ACCORDING TO ^PRIVACY DATA OMITTED^ SHE NEGOTIATED THE LEFT TURN BUT COULD NOT TURN RIGHT, WHICH RESULTED IN HER DEPARTING THE TAXIWAY AND IMPACTING THE TAXI SING. ^PRIVACY DATA OMITTED^ STATED THE RIGHT BARKE PEDAL WOULD NOT MOVE ANY FURTHER AND APPEAR TO BE INEFFECTIVE. INSPECTOR DANIEL R. OSKAR CHECKED THE BRAKES IN BOTH PILOT POSITION, AND THEY WERE FUNCTIONAL. THE NOSE WHEEL STEERING ALSO APPEARED TO OPERATE CORRECTLY. INSPECTOR OSKAR QUESTIONED ^PRIVACY DATA OMITTED^AND HER INSTRUCTOR TO SEE IF EITHER ONE OF THEM WAS AWARE OF THE HORIZONTAL BAR ABOVE THE RUDDER PEDALS IN THE PA-28R-201 AIRCRAFT AND THEY WERE NOT. IT IS NOT UNCOMMON FOR PILOTS TO GET THEIR TOES ON THIS BAR WHEN TRYING TO APPLY THE BRAKES, RENDERING THE PEDAL(S) INEFECTIVE. IT APPEARS THAT THIS WAS THE CAUSE OF THE EXCURSION OFF THE TAXIWAY.
