From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

On 25 September, 2013, privacy data omitted, an instructor pilot for the Maryland state police, was administering an annual check ride for line pilot privacy data omitted, also a pilot for the Maryland state police. The aircraft was a dolphin helicopter.

Approximately 30 minutes into the flight the instructor gave privacy data omitted a simulated engine failure on takeoff. As per established procedures privacy data omitted raised the landing after the aircraft became airborne.

The pilots completed the engine failure emergency checklist on downwind in preparation for a simulated single engine landing. The pilots failed to complete the landing checklist and neither noticed the landing gear warning system activation.

The aircraft touched down on the approach end of runway 11 at the st mary?s airport while the gear was retracted.

Immediately after touch down privacy data omitted had realized the gear was not down and called for engine shutdown to prevent possible aircraft damage.

After completing the emergency shutdown checklist both pilots evacuated the aircraft.

The aircraft suffered minor damage.
