The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.
The left main gear collapsed 5 to 8 seconds after touch down on runway 9 at keyw. The aircraft veered to the left and skidded off the runway and the left wing impacted a runway light. The aircraft came to a rest about 6 feet from a small lake where it impacted a plastic mesh fence. Pilot privacy data omitted was seated in the left front seat and passenger privacy data omitted was seated in the right front seat. There were no injuries to the occupants.
Examination of the aircraft and incident site:
Arrived at Key West international airport (keyw) signature parking ramp at or about 1130 where the aircraft was repositioned to. Observed left gear collapsed, left inboard gear door damaged, left wing leading edge dented outboard of left engine nacelle, left propeller damaged and tips curled, left flap damaged, and right propeller gouged. Observation of interior of aircraft found landing gear handle in the down position, flap handle in the down position, flap motor circuit breaker popped, both propeller in low pitch, both mixture control handles in full rich, and both throttle handles in closed position. Both the pilot and passenger were not present but were available by phone during examination of the aircraft. In addition to my examination, the passenger, privacy data omitted provided photos and a video taken from the right front seat from turn to final till the collapses of the left gear.
On 06/19/2018, privacy data omitted aircraft maintenance technician employed by signature repositioned the aircraft to the signature maintenance hangar at keyw and examined the aircraft. privacy data omitted Reported both via phone and email that the left main actuating rod from the gear motor was severely bent, left flap cable travel housing was bent, left inboard gear door actuating rod was bent, left gear inboard door was pushed into the gear well, and left flap lower side had puncture damage. privacy data omitted Performed operational checks of the nose landing gear, right main landing gear, landing gear motor, gearbox, limit switches, and landing gear indication lights. The operational were normal both electrically and manually. privacy data omitted Removed the damaged left inboard gear door and performed a manual check of the left gear. privacy data omitted Reported the gear was found free to move with no binding in wheel well. The left main gear was manually lifted into the wheel well and released and it fell into down position with no binding. privacy data omitted Straightened out the bent left main gear actuating rod and performed a gear swing both electrically and manually. The gear operated normally both electrically and manually. privacy data omitted Also checked the force to unseat both left and right gear from over-center pivot. Both left and right over-center gear pivot force were found to be equal.
Examination of video:
Examination of the video showed a view from inside the aircraft filming out the front windshield with a partial view of the top of the instrument panel. Video shows 5 to 8 seconds after landing a motor actuation sound followed by the left gear collapsing at which time the video ends.
Interview with pilot:
Interviewed the pilot, privacy data omitted on the phone. I asked privacy data omitted if he has viewed the video provided by privacy data omitted that reviled a motor actuation sound 5 to 8 seconds after. privacy data omitted Stated that after landing he attempted to retract the flap and could have inadvertently retracted the landing gear. privacy data omitted Stated that in the previous weeks, he had flown Cessna?s that have the flap handle were the landing gear handle is located on the Beech D95A aircraft.
