From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

Approximately 10 minutes from 9PN2; the flight medic and nurse stated that the rotorcraft banked hard to the right, and continued rolling, pinning both of them to the ceiling of the rotorcraft. One of the crew members stated that a loud bang was heard directly overhead prior to the rotorcraft rolling to the right. The rotorcraft subsequently descended, made contact with the top of a 40' tree, struck a utility pole wire, impacted the street and bounced, then came to rest on the property of privacy data omitted. A surveillance video also showed the rotorcraft banking hard to the right, rapidly descended and appeared to regain stable flight shortly before ground impact.
