From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

During a routine flight that departed san bernardino airport and was enroute to Chino hills the pilot began to feel a vibration that became progressively more severe until it became difficult to fly the aircraft. The pilot decided to make a precautionary landing at nearby Chino Airport. After landing an inspection was conducted at which time all three main rotor blades were showing signs of severe delamination so it was determined that the aircraft could not be flown back to home base at san bernardino. This form may be revised as needed at a future date.
