From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

Narrative: a northwest DC-9-50 crewed by captain david chiles and first officer clifford hudson struck a paving truck while taxiing into the ramp at fort Walton beach, Florida. Jeff kennedy of the miami NTSB determined that the occurrence did not constitute an accident. The investigation continued as an incident as FAR as the FAA is concerned. This investigation was conducted from the office. The okaloosa county airport police department submitted an incident report to this office. The report indicates that a paving truck was parked in the free zone of the taxi way. There were three inoperative or broken taxi lights in the area of the truck. Safety barrels were stacked to the right of the truck instead of being in place around the equipment. The incident occurred at 2145 on January 14, 1996. The pictures of the site were taken after daylight on the 15th. A statement to this effect is being forwarded. A request to ken mcgurty, northwest airlines assistant poi was made for the crew information. He informed me that the information would be faxed directly from northwest. On February 23, I contacted vance harris northwest airline's DC-9 aircrew program manager in regard to pilot statements that had not been received. He immediately faxed the statements to me. Mr harris related that the cmo investigation of the matter did not indicate any lack of qualification of the crew which would have contributed to this incident. In response to the question as to the captain's last proficiency check, Mr. Harris informed me that northwest airline's training program allows one year between proficiency check rides. After reviewing all the reports and statements regarding this incident, it is my determination that the truck was parked too close to a taxiway and improperly lighted. The airports district office in orlando was notified of the incident and that indicate that they would contact the airport and the contractor to make necessary changes to ensure safe operations. A copy of their investigation or action has been requested.
