From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

(-23) The student was demonstrating autorotations (with power recovery) to the instructor pilot. The helicopter entered the third practice autorotation at approx. 500 ft (AGL) heading northbound. The helicopter was flared and positioned leval at approx. 30 ft (AGL). Attempts to apply power at this stage/altitude were unsuccessful. Both pilots were on the controls at this point. The I.p. raised the collective at approx. 10 ft (AGL) to cushion the landing but the vehicle "fell-thru" making hard ground contact, collapsing the l/h skid, and rolling onto it's l/h side-coming to rest. There were no injuries or fire ensuing. The helicopter/vehicle sustained substantial damage.
