From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

The aircraft was cleared direct to zedag (iaf) at or above 2000 feet. The flight descended to approximately 2000 feet flew direct to zedag, appeared to enter the hold as published but at 2000 feet rather than the 4300 feet required on the charted approach procedure.

Conlusion: the flight crew failed to maintain the minimum altitudes required for the instrument approach procedure being flown, and consequently collided with terrain. (Cfit)

Note: because the crew is deceased, 44709 and eir were not checked in block 32. Other was selected because the company elected to have a safety standown to emphasize CRM, situational awareness, and adherence to flight rules. It is the responsibility of zan to take the appropriate action(s) in regards to the controllers actions, or in actions. See block 43 too.

As the FAA filed it
