8/7/2021 Received maintenance records and pilot training records. No maintenance issues noted. No training records issues noted. Aircraft is recently out of a 100HR inspection.
8/20/2021 At approx. 1430 on 7/31/2021 a C206F registered to privacy data omitted(eqwa) landed at peanut lake to pick up one guide and two other passengers, on a fishing trip, and return them to king salmon Alaska. The 3 passengers were connected to privacy data omitted. It is roughly a 45 minute flight from peanut lake to king salmon. The lake runs east to west. The winds that day were light from the east and were enough to cause ripples across the water. It was a sunny day with 10M visibility. The pilot was flying in from king salmon and landed from the east. It was his first time landing on this lake. He taxiied the lake and parked on the south bank to load passengers. The pilot completed a weight and balance using approximate passenger weights adding 10 lbs to each stated weight. He also used approximate weight for the baggage. Each passenger had one backpack.
The pilot loaded the passengers and began his trip back to king salmon. He lined up on the lake for takeoff towards the east. He lowered the flaps to 10 degrees and began his takeoff roll. When he began his takeoff roll he had approximately 150 yards of lake behind him. He was not using the complete distance of the lake. The airplane was slow to come onto step. As he moved down the lake he lowered the flaps to 20 degrees. He tried to rotate but the aircraft did not lift out of the water. He put in full left aileron to try and lift a float out of the water but it quickly set back down in the water. He attempted the same maneuver again with no success. The pilot moved through the abort point he chose prior to attempting his takeoff but did not pull the power back and abort. He continued the departure attempt. The aircraft struck the bank went almost vertical, stalled and came down on the tundra tumbling over to rest inverted. The left float was ripped from the aircraft and the right float was pushed down towards the right wing still attached at one attach point. Then the evacuation commenced. The pilot exited the aircraft through the pilot door. The guide who was uninjured was able to open the left rear door just enough to get out as the airframe and flaps were hindering the door operation. He moved to the front door and assisted one passenger out of the co pilot seat and then out of the aircraft pilot side door. The third passenger was sitting behind the co- pilot seat and sustained a severe head laceration. Fortunately a nearby aircraft had two qualified emt's aboard and were able to render assistance. The pilot of the other aircraft tended to the passengers that were able to exit the aircraft checking for muscle control and mental coherence. The emt's entered the accident aircraft to stabilize the injured passenger. The two uninjured passengers were moved to king salmon by another aircraft. Branch river flew another aircraft to assist. The two emt's and the injured passenger were moved to king salmon by another nearby aircraft. All three passengers were then transported to anchorage for medical evaluation. They were kept overnight in the hospital for observance and released the next day.

8/20/2021 AT APPROX. 1430 ON 7/31/2021 A C206F REGISTERED TO ^PRIVACY DATA OMITTED^(EQWA) LANDED AT PEANUT LAKE TO PICK UP ONE GUIDE AND TWO OTHER PASSENGERS, ON A FISHING TRIP, AND RETURN THEM TO KING SALMON ALASKA. THE 3 PASSENGERS WERE CONNECTED TO ^PRIVACY DATA OMITTED^.
