From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

Maintenance was performed on the aircraft prior to the flight. The mechanic was called away from the task before it was completed. A screw driver was left on the nose of the aircraft under the wiper and out of site of the pilot. On takeoff the screw driver went into the left propeller and two pieces of the propeller flew into the cabin. The pilot came back around and landed. Called tower to report problem and told tower to check for FOD. Pilot returned to ramp. Screw driver was located on the runway.
