From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.
What happened
The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.
(-23) Conduct of this flight was for the purpose of a commercial certificate check ride. The designated pilot examiner (DPE) stated that during the take-off roll, he was looking out of the right window as the nose of the aircraft rose and then fell, accompanied by the sound of the propeller hitting the surface of the runway. The pilot is a foreign pilot who could not be interviewed because he disappeared immediately after the incident. The DPE and a local mechanic stated that although the pilot denied moving the gear selector to the "up" position, there were no defects found in the system and the nose gear returned to the down and locked position as soon as power was applied with the selector in the "down" position. The DPE thinks that he saw the gear selector in the "up" position immediately after the incident, but is uncertain and the selector was later found in the "down" position.
